Who is the evangelist prophet Dr. Michael Shaw?

Claims to have worked under the late Kathryn Kuhlman, been a Chaplain for the LA Lakers, performed with the Oakridge Boys, and be the first person to play major league baseball with a prosthetic leg? I can find no proof of any of these allegations?
